TruCare Transformation operates as a licensed Mental Health Supportive Living Facility serving two or more unrelated adults with diagnosed mental illness, offering a home-like setting instead of an institution. The organization exists to fill a critical gap for men who need more structure and support than independent living, but who do not require hospital-level care.

Mission: TruCare Transformation exists to provide safe, stable housing for individuals with mental health and co-occurring disorders while connecting them to the services, relationships, and resources needed to heal the whole person and rebuild independent, dignified lives.

Population served: Single adult men with chronic or persistent mental illness, including those experiencing homelessness, returning from incarceration, or transitioning from treatment facilities.

Setting: A group home/boarding home model with on-site supervision, shared living spaces, and clear house guidelines to support recovery and stability.

The home is located at 4155 Brick Church Pike in Whites Creek, Tennessee, on a bus line with parking available, making it easier for residents to get to appointments, employment opportunities, and community resources. Leadership, including contact person Patrick Starnes, coordinates closely with local providers and systems to align services with best practices and community needs.
